1. I understand that prior to my first session, I will receive a verbal explanation of and description of the advisory services to be provided by Jenna Tuttle. I understand that I may refuse any and all services at any time.

2. I understand that the advisory services I will be receiving from Jenna Tuttle are not offered as a substitute for professional mental health care or medical care and are not intended to diagnose, treat or cure any mental health or medical conditions. I also understand that Jenna Tuttle is not, and is not acting as, a mental health counselor or a medical professional. For any and all medical advice, I will seek the advice of an accredited professional, including my medical doctor(s). Jenna Tuttle LLC’s advisory services are not a substitute for counselling, psychoanalysis or any other type of mental of medical healthcare. I represent that I have no medical or mental health condition which would prevent full participation in the advisory services contemplated by this agreement.

3. I understand that Jenna Tuttle will protect my information as confidential unless I state otherwise in writing. If I report child, elder abuse or neglect or threaten to harm myself or someone else, I understand that necessary actions will be taken and my confidentiality agreement limited in this capacity. Furthermore, if Jenna Tuttle is ordered by a court to provide information or to testify, they will do so to the extent of that the law requires.

4. In consideration for the services contemplated by this agreement, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which might occur as a result of the services provided by this agreement.

5. I hereby release, waive, acquit, indemnify and forever discharge Jenna Tuttle, any agents, successors, assigns, personal representatives, executors, heirs and employees from every claim, suit action, demand or right to compensation for damages I may claim to have or that I may have arising out of acts or omissions by myself or by my Jenna Tuttle as a result of the advice given by my Jenna Tuttle or otherwise resulting from the relationship and services contemplated by this agreement. I further declare and represent that no promise, inducement or agreement not expressed in this agreement has been made to me to sign this agreement. This agreement shall bind my heirs, executors, personal representatives, successors, assigns, and agents.
